Instructions for Completing the National Association of Lively Families "Lively Family Sheet"
Purpose: The Lively Family Sheet electronic form is for submitting new, corrected or additional genealogy information to the National Association of Lively Families (NALF). In order to ensure the completeness and accuracy of the information you provide, we suggest you print these instructions and the form and fill in the fields by hand before preparing the final electronic forms for submittal to NALF.
Eligibility: To submit genealogy information for a family group, the following two criteria must be met:
a) At least one spouse of the family must be a Lively descendant (Leibley for the Pennsylvania lines) and
b) The Lively descendant must have born or adopted (notated) at least one child.
Instructions: Upon completing the form, print your work before clicking the button at the bottom of the form to automatically send an e-mail file to NALF (belively@jetbroadband.com).
Notes: The following notes are provided to assist your completing the form correctly and accurately.
- If you don't have data for a field or if a field does not apply, leave that field blank.
- To start using the form, click the mouse in the appropriate ?Type of information? box(s). To move about the form you may either press Tab to move to the next field or click the mouse on the field you want.
- Type of Information: Please check box(s) to indicate the type of information being submitted. Check a box by clicking on it (to un-check a box, click on it again).
If the information being provided:
- a)Is for a family group for which no information has been submitted before, check NEW,
- b)Adds to family group information already submitted to NALF, check ADDS TO EXISTING INFORMATION and/or
- c)To correct information already submitted to NALF, check CORRECTS EXISTING INFORMATION.
- The information fields will expand (to a preset number of characters) to accommodate your information.
- Always give full names; only use initials when the full name isn?t known. When listing names of females (descendant, spouses, mothers, etc.), enter the full maiden name only.
- When you click the arrow of the Military Branch field, a list of the Military Branches will appear on the screen; simply scroll and click on the appropriate answer to insert your entry.
- When you click the arrow of the Lively Line field, a list of the NALF Lively Lines will appear on the screen; simply scroll and click on the appropriate answer to insert your entry.
- Dates: In the form all dates should be provided in the following format: DD MMM YYYY or Day (one or two digits) - Month (three-letter abbreviation) - Year (four digits). (Example: 21 Sep 1896)
- St.: Provide state information in the two-letter Standard Abbreviation format (Example: KS for Kansas)
- Some fields at the top and bottom of the form are for NALF use; please leave these fields blank.
- If you have a question as to what information a field is requesting, simply hold the mouse over the text to the left of each box; a description will appear after about a second.
- If you need more space (for example for 15 children) or have information for the Lively Family that is not requested in the form, include that information in the Notes section at the end of the form. (Example: If the Lively descendant had four spouses; put information about spouse #4 and any children in the Notes section.) If you want to submit genealogy documentation to NALF (i.e., copies of research documents, newspaper articles, wills, photos, anecdotes, family genealogy records, court or census records, etc.), mail them to NALF, 252 Sconset Drive

The NALF form requests genealogy information for a family group in the following order:
- The Lively descendant,
- The Lively descendants first spouse and their children,
- The Lively descendants second spouse and their children,
- The Lively descendants third spouse and their children
- Ties to previous NALF data (in the books) and information on a contact person and data sources, and
- Notes.
NOTE: Be sure to PRINT the completed form before clicking the "submit" button!
